INDEPENDENT CONTRACTOR AGREEMENT
I agree to represent Sigma Tech Communications LLC as an independent contractor. I understand the following:
- I will receive commission/residual payments for referred customers who sign up for services.
- I am an independent contractor, not an employee, and am responsible for my own taxes.
- A 1099-NEC will be issued for annual payments of $600 or more.
- I have submitted a completed and signed W-9 form with this application.
- Commission payments will be made via ACH to the bank account provided.
- This agreement may be terminated by either party with 30 days written notice.
